We are in partnership with Second Harvest Food Bank of Middle TN, Nashville Diaper Connection & Mother to Mother. Please reach out to Connectus Health if you are in need of any of the following items:

1. Food

2. Diapers (newborn to size 6)

3. Essential Clothing and Supplies for Children (ages 0-10)

For the Uninsured/Underinsured, we offer medical care on a sliding fee scale. All patients are asked to complete a sliding fee application based on income and household size. This determines the visit cost.

TennCare, Cover Kids and Medicare are accepted at Connectus locations. Most private health insurance plans are accepted, and if you need assistance with securing insurance, we can help.

Connectus is also available to provide support with getting connected to other community resources for those with financial, housing, or employment needs.
